You will see Amy 8 times over the 12 month membership. You will be fully supported with direct portal messaging, a detailed patient roadmap. All of this and more is included in your membership:

$600

2 Appointments with Amy
75 Biomarkers to provide a broad range of insight into what's happening inside your body.
1 Comprehensive Action Plan to review those insights and options for treatment.
The Functional Medicine Checkup is a great way to get a deeper understanding of your current health. Please note: care beyond the two CFM visits is not included. If you choose to join as a member, the entire cost of your checkup will be applied to your Membership fee.

FUNCTIONAL MEDICINE CHECKUP

How it Works

You start with an introductory consultation with Amy. She will learn about you and order a comprehensive panel of blood tests.
Our panel of tests gives you a comprehensive set of 75 different markers.
Amy will then review your results with you to:
- Discuss the importance of each marker and how to use optimal functional ranges to best understand the inner workings of your body
- Provide personalized recommendations
- Present your plan with actionable steps for you to take
